Dr. Fulks' Reward Program

You will be competing against yourself for prizes. Here's how it works:

You will need to collect assistant's signatures by following "THE RULES."

The more reward cards you collect, the more you win! To completely fill your reward cards all you need to do is simply follow "THE RULES."

If you follow all "THE RULES," you will receive the appropriate number of signatures from the assistant who works with you at your appointment.

THE RULES

• Be on time for your appointment
• Receive a good brushing grade (A or B)
• Have no loose or broken brackets or bands
• Wear your Smile Construction T-shirt to your appointment
• Schedule appointments between 9:30 a.m. and 2:00 p.m. when school is in session (This is worth 2 assistant signatures)

You will not receive an assistant's signature if:

• The appointment is an emergency visit
• Your appointment is to replace or repair a broken bracket or band
• You receive a poor grade on your brushing that day

You are solely responsible for your reward card. It is just like money, if you lose it then it is gone. Once you have a completely filled card you may turn it in to receive a gift card. You can trade in one card for a $5.00 gift card. Or you can turn in two cards for a $10.00 gift card.

Gift cards are available for Regal Cinemas Theater, Starbucks, Target or Walmart. You may also trade the appropriate amount of signatures for something in the toy cabinet.
